It looks like the issue are caused in the context menu of Explorer's
shell extension.
So if possible, could you try "DbgView" and send the debug output?

You can download it from this site:

1. Run "DebugView" application
2. Open TortoiseHg's context menu in Explorer
3. Select any menu item
4. Copy & mail the debug message in "DebugView"
