Although far from perfect, my suggestion is as follows. Since there's nothing
preventing the title from having a slash (or any other character) in it, it'd
probably be better, when -g is used, to allow an optional second parameter
which would be the category. If not specified, select the first category in
which the title appears. Even with multiple categories, the titles will
probably be mostly unique anyway. They're used to create screen menus. Listing
the bookmarks should probably also be category-based, or, at least,
category-influenced.

You could, at the risk of being non-standard, introduce a "Shortcut" variable
for this. If you just use single characters, then, while reducing typing, the
need for screen reviewing will be significantly increased.
Even with full names, the user could, of course, just use single letter names.
Another thing you might try is allowing unambiguous truncations of the full
names. This way, if the user carefully picks the names, single letters will
work anyway.

The standard (which you probably know already) is to use whatever $PAGER is set
to, and, if it's either unset or null, to use "more".

That's too bad. It's incredibly useful.
Another way of scrolling bacdkward is to use screen. If, within a screen
session, one enters ctrl-A followed by a left bracket, copy mode is entered. In
this mode, one can enter vi-like commands to move around within the session's
buffer.
