You got this one backwards, it's for 8.6 or greater.
Adapter Card Support 1.4.1 was released on 4/4/00 (same day as
9.0.4); nearly a year after 8.6 was released (5/10/99).
You should be able to do a custom install of just USB and Firewire
support from your system CD, or re-apply the 9.22 Combo update.
The 9.2.2 USB extensions (v.1.5.9) are known to break the function of
many USB cards, including all combo FW/USB AFAIK. There is a
technical explanation of why these dual combo cards get broken under
1.5.9 extension, but I didn't understand it. I just followed the
instructions on how to install 1.4.1 and still retain the Mass
Storage Support extensions from 9.2.2. Worked for me on two different
combo cards with two totally different chipsets. Both failed USB in
9.2.2, and both worked under 1.4.1 after I'd read the solution. My
cards were the OrangeMicro with NEC USB chipset (USB 1.1); and a
generic with VIA chipset (USB 2.0, works as 1.1). Both worked fine
under OS X.
