Thanks, I have sent the document to you off-list.

If you open it in MS Word you can fill in the boxes without any error messages.

If you open it in OO Writer and start to fill in, say your name, you get an OO error message that the document is readonly and you can't change it. Then go to Format/Sections. You will see that the document is in two sections, each of which is Write Protected. Unprotect each section and you will be able to fill in the form just like in MS Word.

When I am OOo, I can click on the grey shaded areas. Then a pop up box appears. I can type in that (eg. my name) and when I hit okay, it 'fills in the form'.

I have not 'unprotected' anything. Is this not a workable solution?

The only issue would be that you can't tab between the form fields.
