Qwest offers 256Kbps up/down for $15/month according to their web site, no activation charge and you can rent a modem for $3/month. They also have 1.5 Mbps/896 Kbps for $28/month.
I had qwest DSL + xmission for about 5-6 years and it was solid. You don't get the pokey connection speed that way -- if you use MSN (via Qwest) then you will complain about the slow speeds. If you use Qwest + xmission you will be very happy. Xmission is $19 a month, but that includes shell access on reliable well-configured servers. Xmission also gives you quite a few "upgrade" options for your DSL bandwidth guarantees, etc. I would still have this setup if I could get DSL at my new house.
We have just started using Covad in Chicago (for business) and I've only had one issue with them losing a static route to us. So far, so good.
