I've opened this SE/30 so many times that I need no more than a minute, these days.
The numbers on the chip, from top to bottom: TMS 4461-15NL HHP 9022 9091S SINGAPORE Next to this, a logo is printed with a T in it, so I'm guessing that TMS is the name of the manufacturer.
