THe home button ineffectiveness I have noticed more since upgrading to i.os. 
I'm glad I'm not the only one since I was beginning to worry that I had a 
faulty iPad 2 and would have to take it for repairs which is usually costly. 
Have you by any chance reported it to Apple? 

Also, do you know that you can reset Voiceover by pressing the lock button 5 
times quickly? That might save you going into settings.

The 5 finger pinch has to be enabled in settings/accessibility/voiceover. There 
is also a 4 finger swipe there which you may find useful on the iPad.
