Hi Sandra I have just recently put an app on my phone called Emergency Ice. This can be used in different countries and a lot of information can be put into the app in case medical details are required. I can't remember if it was free or not, but it wouldn't have cost very much if I downloaded it.
I'm pretty sure I've got the name right, perhaps you could put the word ice in to the app store search.
