Hi Stephanie, One way to tell whether your phone is on or off is to flick the Mute switch above the volume keys. If you move it towards the back of the phone which means you are turning on Mute, the phone vibrates briefly. If it does then it's obviously on and you can flick the switch forward again, it doesn't vibrate when you unmute.
If you have a landline or use Skype you could always call your phone, too, and see if it rings. Now, as for Voiceover, it has probably somehow gone to 0 volume. I would first plug in your wired headset and see if Voiceover works. If it does and then it doesn't when you unplug the headset, make sure you unlock the phone using the headset. Now unplug the headset and do a 2-finger swipe up on the screen. This makes Voiceover start reading the content of the screen starting at the top. Now repeatedly press the volume up button to raise the volume of the external speaker.
