Apple Mail doesn't have an Address Book as such. When you click Addresses,
you're only getting a "glimpse" from the actual Address Book.
Anything you want to delete/change/modify, you have to do from the "Actual"
Address Book.

Open Address Book. (From Applications folder, not from within Mail)
>From All, highlight the name you don't want and then hit the delete button.
Similarly to add Groups, it's all done from the "real" Address Book.
That's it.
