Hi Mike,

I think I now understand what your problem is.
I am using Acrobat 8.1.5 in Snow Leopard. A PDF opened in Acrobat Pro, I go to 
File > Page Setup - it shows, Format for: Any Printer, 
but I can then use the arrows to select my Epson TX700W printer or any other 
printer I have installed and added in Print & Fax.

You are not able to select any other printer, as no other printers show under 
"Format For"?

Mike try this:
1. Open Acrobat Pro first
2. Then go to File > Open to open the PDF in Acrobat

Then can you now see your Printers in the Page Setup?
