Thanks again Ronni and also James for your suggestions.
I'm not sure what happened, and I may have subconsciously done just
as you suggested Ronni, but now Acrobat is showing the printers in
the Page Setup requester. This after almost 3 days of trying all
sorts of things to get it working.
Now I can print my required size using Acrobat - I hope it lasts.

To answer your questions:
Yes, the Epson R230 shows in the Print and Fax prefs, along with
my other printers.
To elaborate - I have no problems choosing the printer when
printing from Acrobat, or anywhere else - it is only that I can't
choose it in the Page Setup (in Acrobat only) which results in a
reduced print area available because of the "Any Printer" default
being used.
I can print from Preview OK - just that Acrobat has more output
options except, in this case, the printable area of a page. In
fact I can print OK from Acrobat except the aforementioned problem
- I can reduce the print size to fit the printable area but that's
not what I want with these files I'm printing.
I have seen someone else with this problem http://

"At times, you may encounter problems when printing a PDF file.
For example, a PDF file can contain damaged content such as
images or fonts that cannot be rendered by Acrobat during the
printing process.
Choosing Print As Image bypasses that rendering operation by
sending the printer a rasterized image of the document.
To print as image:
1. Open the PDF file in Acrobat.
2. Choose File > Print.
3. Click Advanced.
4. Select Print As Image near the top of the dialog box.
5. Click OK, and then click OK again to print. "

I'm using Tiger and attempting to print to an Epson R230,
although it doesn't matter which printer I use - it is the Page
Setup that doesn't allow me to choose any printer because none
of them are listed in the "Format For" selector.
On my wife's MacBook, using Snow Leopard, the Page Setup shows
all printers so it is just this version of Acrobat on this
computer that is the problem. I could get around it by moving
the files and printing from the MacBook but I was hoping there
might be a simpler solution, such as trashing a preference or
something.
