Thank you, Ciz.
DEFAULTSORT has changed, I think. When I previously read about it, I got the
clear impression that a new DEFAULTSORT on a page would apply to all
subsequently-listed categories until replaced by another DEFAULTSORT lower
down. Not now.
So for what we want (listing by PAGENAME in the relevant surname category but -
as is normal for sorting people - by surname in all others) it seems we should
have the normal DEFAULTSORT right at the bottom of the page (so that it's not
overriden by any later ones hidden in templates) and use a specific sort order
pipe for the surname category.
Trying just now to update the notes on Template:Surname, I get an amusing error:

mailing list. Some kind person tweaked something
so that the other reported malfunction of
