If you'd like an onsite solution, look into Freeside or Optigold for the billing portion. Both of these are reasonably priced (if you are familiar with Linux, you can download and install Freeside for free). Both have the ability to do provisioning for new users.

For monitoring, there are lots of options out there. I really like the the one that Mikrotik has released called "The Dude". It is free, and if you are using Mikrotik within the network, it will offer a TON of "extra" features above and beyond just simple monitoring. If you want to spend more time, you may find that Nagios is a better solution for you. I liked nagios, but for me The Dude really outshined it. Also, there is a program called Cacti that many people really like, but it is not so much of a "monitor" program as it is a graphing program. In other words, it will create graphs of bandwidth usage, signal levels, error rates and anything else that your system makes available via SNMP.
