This often bites me. I've learnt that when pasting won't work, it's because of schema rules, which changed between DocBook 4.2 and 4.5 as well. Then I go back to read the DocBook docs and more carefully select what and where to paste. It would be wonderful if XXE tried to help, e.g. by prompting "Paste <clipboardelement> before or after <some-ancestor-element>", or perhaps a dialog with "Wrap in [pick from list of allowed parent elements from the schema] before pasting?"
