Hmm, personally I found this pretty self-explanatory. Just create a template, set the command template as something like "/usr/local/zenoss/common/libexec/check_foo -flag1 -flag2 $devname" and create a datapoint for each value, like "RTA" and "Packet loss". The name of the datapoint has to be exactly the same as the output of the script.
