Well, assuming that no one actually deleted the form, open the Remedy Support form and re-enable the Entry Point to that form in the View Properties. That will make it visible on the Home Page. Then inform that person that they owe you (lunch, drinks, chocolate, etc.).
If they DID delete the form, they owe you BIG time to restore it. Then yank their Admin privileges away.
