I had this issue and the problem is related to application permissions. The usual fix for this is to modify (not simply open) the application "Remedy Asset Inventory", where all AST forms reside. Your new class forms will likely not be included. (If they are, remove them.) Modify the application form list by adding your forms, then save the application. This should apply the needed role-permissions to all the objects on your new class forms. Note: This takes a lot of CPU, so be careful if doing this on your production system!
