We have been experiencing this a similar issue Started after upgrade from 6.3P20.
Issue reported: 11/15/11 07:43:23 Environment at time ARSERVER = 7.5.00 Patch 004 201002051027 OS: UNIX 6.1 ORACLE = 10G JAVA - 1.5 WEBSPHERE 6.1 Java webshpere = 1.5 Mid-tier version = 7.5.00 Patch 004 201002051027 Custom Environment. On the Midtier, on create of new ticket Change (does not occur with Service/incident, just tested) with attachment as a non-admin, the attachment is not saved within ticket. You have to re-attach attachment after saved request. This does not occur if the user is an admin. The occurs in all version of internet explorer and mozilla. At the time: re-added permission to attachment field , save the form, then removed the permission added then re-add original permissions. Then tested adding the attachment to on create in the web as non-admin and that worked.
