Mac and iPhone are related, but not in any practical sense--they are two different products with two different operating systems that share a few high-level and many low-level features in common with each other. Now, to your questions:
1. Sorry, no, you can't use Mac voices on iPhone.
2. Yes, apps permitting.
3. Batteries have never been replaceable in iPhones. This usually isn't a problem, and frankly given the life of these things I've never had a problem with it, but you might want to be careful getting it second-hand.
4. If you can upgrade the iOS software, and you usually can except for the oldest of phones, then you get the new features in VoiceOver between releases. Yes there are often new features. Right now, the oldest phone you can get to run the newest software is iPhone 4, but that's now quite slow by today's standards.
