Put who on hold?
If you put PSTN on hold that would work fine but if you put IP phones of other sites on Hold then they will not hear anything. This is because IPsec limitations over VPN (this is also mentioned at Proctor labs login page).

To check if your MOH is working fine then you can run the folliwng commands and check the outputs.
These are the outputs I can see for my BR1 MOH:
