So you are saying that a good starting point would be to convert all the PDFs into forms?

Having gone back and read the entire thread, my first question to you would be are the pdf's already forms? I kind of implied that them might be when you said you don't know "why the doctors don't fill them out, print them and then fax them."
If you don't have to go through the trouble of creating pdf forms out of pdf documents then you may want to lean toward a solution that builds on this starting point.
If you are going to have to turn pdf documents (that look like forms) into electronic compleateble pdf forms than the work involed in that may lead you into another solution such as creating them as crystal report forms or html/css forms or something.
