Let me reach across my desk and get my copy of it. Yep its called the DSM-IVR. 
Published in 2002 by the American Psychiatric Publishers

Nope no tests came with it - Just a very thick paperback book with a silver 
cover - I was too cheap to get the hardback with the maroon cover.

You said the test was called the DSM-IV, to quote:

I really think you need to check back with your sources and get the real name 
of the test. It may be based on the diagnostic criteria listed in the DSM-IVR 
but its not the DSM-IVR. That's just a set of guidelines for diagnosis. What's 
more, as a whole its a compromise cobbled together by a set of committees. And 
we all know what committees are like, at least 6 legs and no brain.
