I had a look at some options. All of them are either paid or very
limited in the free version. I would love to use Limesurvey just like
the ASF but getting the paid version is probably too much effort for our
small survey.

I drafted the first version on Google Forms:

The answers are very conveniently available in a public sheet so we can
provide transparency what is recorded:

@privacy Is a Google Form with these questions problematic? Can we use
it if we point the user to Googles Privacy Policy?
