Hello,

I looked at the questions and also the spreadsheet. I don’t see any issues.

When you promote this survey make sure users know you are sending them to 
Google (before they click). Bonus points for adding the privacy terms.

As long as this form is community driven and optional, it is fine.

Let me know if I should take another look at some point- otherwise good luck 
with the survey.
