In the Customize dialog, set
the "Save to" list to the template name (rather than Writer) *first*,
then do the customizing.
Good tip.
In the Save As dialog, don't trust the
"Automatic Extension" checkbox when saving a template, even after you
select the proper (*.ott) extension from the list.
To save a document as a template, it's better to use File > Templates >
Save, which opens the Templates dialog and ensures you save the template
in a template folder, and also takes care of the extension.
