Could it be that the Boot\Grub area is corrupt, hence once out of grub
the disk acts as normal??
Where possible I try to isolate the disk I am testing especially when
chasing an issue, so the OS is running also where or if you are using
disk manufacturer test software that is all running from memory\ramdisk
so the hard disk is only being accessed by the testing software.
You could try Rescatux & Super Grub2 (Http://www.rescatux,org) to see if
that can ID any issues in your grub

Looking at the images, to me it seems that your SSD is playing up. I
would boot from a live usb and then run some smart tests, running
them two or three times on both disks. While you have the Live USB
you can mess around with the PC running from the USB to test other
aspects of your PC to ensure it is running OK. Also check the SSD
manufacturer web site as they may have some test software for you to
run and test the SSD.
As stated in my follow-up message, I can actually get into the system
after exiting Recovery Mode. I included the results of the SMART
tests, which show nothing untoward. These results are from running
the tests via KDE Partition Manager; is there a better way?
