That may be a 9.0.4 bug with some applications (my daughter experienced 
the disappearing icons with AppleWorks - when she quit, or force quit, 
AWks, all the contents of the AppleWorks folder disappeared!). I 
understand that upgrading to 9.1 or later helps (am looking to upgrade 
her iBook to 9.2.2 over her Christmas break from college).

Others may be able to give us more details on this. I also understand the 
9.0.4 "volume wrapper" bug is related to this, and upgrading your copy of 
Apple's Disk First Aid and running it on your drive will fix (at least 
part of) it.
