I've never tried to repair anything since going to OSX, and I'm

I don't even know what an extended attribute is. Is it serious?
Should I boot off of the install disk and repair? Is there any way
to screw that up?
Yes, boot from another disk and run Repair in Disk Utility. Or use
your favorite disk repair tool (Tech Tool, Disk Warrior, etc).
Another option, and something that you may want to install anyway, is
to use AppleJack. You install AppleJack and then you can reboot into
single user mode (Command-S at boot), and once in single user, run
AppleJack and it does a bunch of maintenance tasks for you. Avoids
the need for a bootable CD to run Disk Utility's repair feature.
