How do I do that? Does Mail preserve my folder structure and the items

That would be a nightmare. I have 10 years worth of filed email messages
on various topics and probably 120 folders plus subfolders.

It used to be as simple* as choosing Import from the File menu in Mail and telling it you are importing from Emailer. I just looked and the version of Mail that comes with 10.5 no longer lists Emailer as an option. I don't know what version you are running and if it comes with support for importing from Emailer.

*note: simple = that is all it was supposed to need. In reality many people have had some problems getting it to work, although in following all the stories, the key seemed to have been to either do it a few folders at a time, or just be REALLY patient as it can take an amazingly long time, as in more than 24 hours to complete if you have lots of folder and lots of email.

There is also a 3rd party tool (don't remember the name, but I'm sure someone will speak up) that I believe can open and read the emailer database and directly convert it to the Mail stored mail structure.

Finally, there are applescripts for emailer that will let you export your email in standard mbox format which Mail can import. Downside to doing it this way is you pretty much have to do it folder by folder and recreate your folder structure as you go (there may also be other side effects as I believe some of the mbox exporting scripts failed to properly preserve dates).

As for maintaining the links to your attachments, that probably won't happen. Emailer stored the links to the files in an odd way and even it would lose track of them from time to time. I'd be willing to bet the attachments will not remain connected to the email. In fact, I'd place money on it, because Mail (as do most other modern clients) do not detach the attachment like Emailer does. Rather they keep the attachment embedded in the email itself and you save it out as you want it. Thus Mail simply has no mechanism to retain the file attachments that Emailer uses.
