I’ve used the following format, not dissimilar to EU DoC, which seems to cover requirements in 47CFR2 subpart J : We [Company Name] (of Manufacturer or Importer, which must be located within USA) [Company Address] (which must be located within USA) declare under our sole responsibility that the products: [Product name and number] to which this declaration relates are in conformity with the following regulations 47 CFR 15 [Signature] [name] [title] [date] Don’t know if it’s “right”, but never had any complaints J Regards Charlie From: McInturff, Gary [mailto:[email protected]] Sent: 20 September 2010 21:20 To: [email protected] Subject: [PSES] FCC DoC I think I’ve gotten a little sloppy in my documentation. The FCC back in about 1982 had a Declaration of Conformity very similar to the EU’s CE Declaration of Conformity, but I haven’t seen it used or used it in some time. Rather the product carried the required part 15 markings, and their was a heading in the manual that said FCC DoC that simply stated it was tested and passed Class A or B. The user warnings about moving things around or modifying the thing were in that section as well. Over time I had completely forgotten about the FCC DoC. Are these still being generated and can anyone give me an example of this, as I remember its an internally generated document one you got the official test complete at an approved lab. We generate all of the CE Documents, when appropriate, but this is the first time I’ve had a customer request one. We did send the test report but they are insistant that we give them a FCC DoC. Happy to obligue but would like to find the reference CFR 47 part 2(?) and an example if someone can share.
