With hardware counting, it would be only for very slow speeds that emc
would see every count (one edge per servo cycle).  So, the kind of
fault detection you describe would have to be in the hardware.

Jon's idea for an "offline" test is good (you'd just have to run it
slow enough) and I've actually done that.
