There are a couple of answers that I know.
Some encoders have a movable magnet that strengthens the signal.
Heidenhain did this in some of their's.
You would see a regular interval signal though with one spot stronger.
For acurite / anilam, before scanning the index pulse, you were asked to 
preposition
the machine. This way they could have multiple index pulses but only use one.
While in regular use the DRO would ignore all index pulses
Some heidenhain did the same sorta thing though they only used two index spots,
close to each end if I remember right.
