OK, I stopped the Groupshield service, the problem remained. Email flowed as
expected to other external destinations OK, yet to this particular domain
(@dial.pipex.com) it queues up. Then after a several mintues the odd mail
gets through again.

When I restarted Groupshield the symptoms were the same, mail queues up,
same error message. Then suddenly a few email get through.

I'm completely baffled!

It is worth getting any AV software out of the way just for a quick test
(not suggesting that you run production systems without AV!) at least it is
something you can then take out of the possible problem category.
Give it a try.

No, they don't all have attachments. 

I initially thought there must be a problem with the external companies SMTP
service, but then realised the timeout error message was being generated by
them and not me! Therefore my SMTP virtual server is sitting there at the
DATA prompt doing not a lot, and then getting timed out by the remote
server.

I forgot to add I am using Groupshield 6.0, could this be interfering?

Over the past few days I have noticed that for one external site, SMTP mail
regularly builds up, introducing delays of up to an hour before delivery.

I have checked the SMTP logs for the Virtual server and can see that the
connection is timing out:-

The SMTP Virtual Server is working fine for other external sites, so I'm a
little confused!!  The mail entry is marked up for a retry and eventually
goes through.  I have manually connected to the external site in question
using Telnet and the remote system is performing as expected, there are no
excessive delays.

Can anybody shed any further light/suggestions?
