Then I'd do as JB suggested and delete the account (don't save any data
if it asks) and reactivate it. If nothing else it may narrow down the
possibilities.

No Outlook rules - just checked.  Device is set to Mailbox Wins on
conflicts.

Outlook to "win" on conflicts?
