Yes, xfree86 is basically the only package allowed out of the /sw sandbox.
I think emacs might put some stuff in /usr also, but most other things
should not be allowed to do this. Basically, X11 is more or less hard
wired to be installed in /usr/X11R6, and "porting" it to a different base
directory was just too frustrating to sort out. Therefore, allowing it out
of the sandbox was a calculated move & is generally not used elsewhere.
