Lincoln, I haven't chimed in on this because I was having similar problems. I had to 
reinstall X when the partition became unbootable on my G3. I attribute this to an 
update, probably the 7-12-02 Security Update that Marc alluded to. Here's what I did 
and it may help you.

I don't remember if you're installing on a partition or a separate hard drive. If it's 
a separate drive initialize it. If it's a partition delete everything on it. Then 
highlight the partition and erase it. Install OS 10.1.

To install the 10.1.5 combo you only need two updates. They are, Security Update 
10-19-01, install this first and restart the computer. The second one is Installer 
Update 1.0. Install it and restart. You should then be able to use the 10.1.5 update 
combo. The only drive that you can select will be the one with 10.1 on it.

I will never install another Apple update for OS X until I get feedback for it.
