Great Michael, You did get the PDF form to work in valid format for data importing to Google Health. Can you share them here?
Do you mind to explain how you construct the PDF? Did you use Full version of Acrobat and JavaScript to create form fields and validate data fields? Do you upload XML data file to Google Health account via web interface of user account?
