There are many online tools that offer online forms. I do not think anyone needs to contact Google to ask for such things, as they already exist. I happen to like PDF Forms because. --- PDF Forms can printed filled out with a pen to bring back to the practice --- PDF. Forms can be filled out with Acrobat Reader and printed to bring back to the practice --- PDF Forms can be filled out online with Acrobat Reader (or in a browser) and submitted back to the practice. I created a silly web site for a demo that contains several forms that can be filled out online and submitted to my system. http://www.myhealthcarestuff.com/dev/ there are links to two non PDF forms on the very top right. I used Surveymonkey on these examples. http://www.surveymonkey.com/ The PDF form is taken down to be editied, but i can have it back up once I finish the explination on how i collect and share that data.
