@ Bess Ho

hey, totally off subject, but as you are an iPhone developer, you should
take a peek at voalte.

The PDF form lets people type into it.

The PDF form exports a valid ASTM CCR XML data file.

That is where I stop - I am looking to others to figure out how to consume
that CCR data file.

The construction of the PDF is using the things you can find here

http://www.aiim.org/standards/article.aspx?ID=33284

- and in our case, with Adobe LiveCycle -
http://www.adobe.com/products/livecycle/

Question - Did you use Full version of Acrobat and JavaScript to create form
fields and validate data fields?

no. This uses XFA forms.

Question - Do you upload XML data file to Google Health account via web
interface of user account?

No. That is what I would like to have help with.
