It's usually used to refer to a straight guy with an urban fashion sensibility, the kind that's usually associated with gay guys. Metrosexuals care about fashion, style, colour, skin care, and don't think it's unmanly to go to a spa.

Henry VIII would only be a metrosexual from a modern standpoint. In 16th century England, the upper-class men were as much or more into fashion as their female counterparts, and this was not considered strange or unmanly.
