I vaguely remember having this issue on an old CSS box. It ended up being caused by a dodgy SM plugin or something to do with it's configs. I'm sorry I can't be more detailed but I've not experienced this for many years. Obviously being from BestGN I won't say you don't know what you are doing, but have you tried a barebones install, then just ran the server using ./srcds_run -console -game cstrike +map de_dust2 and see if that still fails?
