Little update,

Ran a ping test, lost only 3/1000 packets.

I rebooted and turned absolutely everything off on the box so HDD / CPU /
Memory usage was ~0%, had the datacenter last night install a new fibre
link.

Still the same thing happens, only on TF2.

Everytime it warps loss goes through the roof, sometimes the timeout
reconnecting thing appears in the top right.

I'm out of ideas,

Very strange.

Try running a simple network ping in a loop once per second (and log this
somewhere), so you can see if there are any network problems during these
spike periods (perhaps the other games handle it more gracefully?).

Also try graphing cpu/memory usage of the game server (and the host itself),
maybe its spiking in CPU for some reason which would explain the spikes (if
its busy processing some data/attack).

Here was a extreme case with 24 players, it randomly happens. Soometimes
might be fine for a hour then it spikes.

http://files.bestgn.net.au/cp_dustbowl0092.jpg

Looks like a timeout,

I sat down on a empty server with a stop watch, had mini spikes following
durations apart

(empty)
- 15sec
- 22sec
- 10sec
- 14sec
- 25sec
- 50sec
(server fills - larger spikes)
- 28minutes
- 10minutes
- 63minutes
- 16minutes

The ones seconds apart where little tiny jumps, the minutes apart jumps
where bigger. The biggest one ive seen is in the screenshot.

Which part does it spike? The very first part (0 on x-axis) or further along
in the middle?

What you should do is sit idle in the server and note down when the spikes
occur for 5-10 minutes and report back, lets see if there is a pattern.

Thats what happens when it spikes, no real time pattern between the spikes
seems quite random. Tried both on plain / custom, clean, old installs and
copies from the other game server seems to make no real difference.

No NTP either so its not clock drift :/

Another thing to try is while playing (be sure it's a stock server -
stock/plain servers should have zero problems!) turn on net_graph 3 and
watch the output when you hit the problem.

It could possibly be something within the data centre (maybe some device is
temporarily holding game packets hostage for a second or two, which would
explain the sudden warp)?

The net graph should hopefully show this (sudden drop of activity and then
spike).

Tried it all, with and without sourcemod /metamod. Had a few linux devs look
over the kernel etc.

TF2 Servers are holding perfect 990fps and 0.2ms VAR, cpu usage basically
nothing. 100mbit link using only ~10%. Last night I played with
net_maxcleartime, net_splitrate and net_splitpacket_maxrate managed to tweak
it so we get 0 choke yet no difference -.-

I also tried stock fps 300, made no difference. I also expermimented with
ping boosts.

I can run left 4 dead on the box without any issues. The issue seems to be
TF2 only :/

On my other machine, no issues what so ever the machine is literally sitting
one row above in the datacenter. The machines are running cloned setups. Had
memory replaced, tried different cpus / cores etc.

I vaguely remember having this issue on an old CSS box.

It ended up being caused by a dodgy SM plugin or something to do with
it's configs. I'm sorry I can't be more detailed but I've not
experienced this for many years. Obviously being from BestGN I won't say
you don't know what you are doing, but have you tried a barebones
install, then just ran the server using ./srcds_run -console -game
cstrike +map de_dust2 and see if that still fails?
