I have one customer who has this problem only on his home computer. He can send and receive mail fine from his work computer. This has always led me to believe that it was an issue with his particular version of Outlook Express. After the same sort of exhaustive testing we told him to upgrade his Outlook Express to the latest 5.5 release. Of course he won't because he is stubborn and of course blames everyone else but himself. So I don't really know if the version is the problem but you may want to give it a shot.
