Unfortunately this is another one that spammers have ruined for
everyone else.  The chain of events goes something like this...
Spammer signs up for a trial account, blast away spam which is relayed
off un-suspecting third party servers.  Complaints to the ISP flood
the abuse department, spammer gets knocked off line.

So a dial up provider is essentially held responsible for their subscribers
actions in this case.  And the only way for a provider to be able to
control the actions of that luser is to force them to use a local SMTP
server so they may at least monitor the amount of outbound traffic
that is being pumped through and also be able to catch the spammer
in the act versus responding to thousands of complaints from others.

Who knows what the best solution would be, however I would say
a start would be that server admins be required to manage their
MTA in a responsible manner and prevent anonynous third party relay.
Since those un-suspecting servers that are open for relay are the very
ones that the spammers will target.  So if all the servers were secure this
would be a magic answer.  However with so many clueless admins
floating around this would be near impossible.

So it is actually cheaper for the ISP to take matters into their own hands.
Although that is not the most desireable either.
