2. and 3. are handled wonderfully by my IMGate project, which uses postfix 
on FreeBSD as a front-end for Imail, as an anti-spam defense just about 
every which way possible, fully configurable.

postfix, an IBM public software product by Wietse Venema, also supports 
full RegEx filtering of headers for global filtering before relaying to 
Imail, where domain level filtering can be implemented.

postfix also can off-load ETRN traffic from Imail, with no polling while on 
hold (still needs a remote fixed ip address, tho').

postfix also has "delivery rate controls" because postfix is a high-volume 
multi-threaded transfer SMTP client that appear to be  a "thundering herd" 
to a overwhelmable SMTP server.

And of course postfix can deliver all outbound traffic relayed from Imail 
at highspeed, freeing Imail to concentrate on POP3, IMAP, LDAP, dial-up 
clients, and web messaging clients.   We are finding the Imail + IMGate 
combo to be extremely effective combination, esp when IMGate is completely 
free (old P200 + free software).
