Usually it means the first 'opt in' you were slammed into the list without your knowledge or permission then when you get the first UCE you have the option of confirming or unjoining (thus the double opt-in language).
Email is a person-to-person medium based on prior consent. It is not a broadcast medium. I support SpamCop and everyone trying to save our Internet Email system from those abusers who shift advertising costs to the end recipient.
