When smtp down, sometimes port25 not accepting connections, sometimes it connects but didn't receive welcome mess.
You have confirmed monitor's "opinion" by telnetting to port 25? ie, IMail SMTP responds fast/slow and/or differently when relay-for-everyone is on or off
The other SMTP player here is your SMTP gateway. Does its logging per connection (if it can log with time-stamps per connection), show any difference in SMTP-to-Imail-SMTPD session times when IMail relay-for-everyone is on or off?
Relay for every would mean that Imail SMTPD would never have to call the SMTP AUTH routine, nor lookup local hosts/users or IP-subnets. If that overhead is really what is causing your SMTPD performance problem, then I'd say your machine is just out of resources.

ok, so you say the monitor "problem" is fixed with relay for everyone, and caused by not relay for everyone. I suspect the monitor as a false problem, but interesting anomaly...

This is probably reading huge .mbx file, or de-MIMEing a huge attachment.
Most of the time, total cpu usage between 60% and 20% every 5 sec
sounds pretty busy. I'd say, if you don't find a real problem, that you're looking at a hardware upgrade in the medium term.

this is bug in POP logging, ugly but harmless. turn off POP logging, turn off ALL Imail logging except for SMTP
I will examine the smtp log also, what should I look for? (daily 70mb log file)
The SMTP log will be mostly useless since it is only connecting to/from the SMTP gateway for Internet traffic.
how many SMTPD or SMTP processes in memory?
The problem with SMTPD not responding could be do to hitting a max-process limit, or it could be that the machine hit a swapping threshold and starts thrashing the disk.
So what about swapping behavior? A mail server should never swap, but Windows apparently is a trigger-happy swapper (we can always trust MS to write software that will respect and conserve our resources and our money as finite), even when memory is freely available. I can't find where you said how much memory you have. I'd say 1 GB is minimum, maybe 2 GB. I think there are some registry tweaks to make Windows less
While 2 x 1.2 GHz should be enough, you might look at upgrading to much hotter hardware, or setting up IMGate to block some of the 1 Mb/sec SMTP traffic, which must be 50+% junk.
