I have no idea how these things are being used in your setups but the only failure mode I've ever seen in them is a "short".  I only have to see the sparks when placing the charger clamps on the battery on the test bench to identify the problem.  No need to go any further.  Take off the cover, replace the rectifier, and send it back out the door.
