I've seen more than one fail but I recall them being maybe a 50 amp rectifier and running at 20 or so amps max charge rate for hours on end.  Even with those numbers I don't see a lot of failures but when they do it doesn't  take long to troubleshoot. If your application runs at only 25% of that load you may never see a failure but anything man-made is capable of failing.  That's why I have a small 4 AH backup battery on my fuel pumps.  It was only needed once but when I needed it it was there.
